Background
Medicines reconciliation is essential in ensuring safe prescribing of medicines during a patient’s hospital admission.
Methods
A prospective clinical audit was conducted within the Medical Receiving Unit and Acute Medical Receiving Unit of Monklands District General Hospital using a newly developed printable Emergency Care Summary Medicine
