Background
The PharmaCloud System provides patient’s medication records for the past three months. In order to improve the quality of patient-centered medical care, we assign responsible pharmacists to the integrated care clinic who survey the rationality of cross-hospital medication by checking the PharmaCloud System with or without in-face.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
