  The Health Hypertension Passport (HHP) focuses on empowerment, and increase in patients health education, motivation and dialogue between care-partners. Beyond transmitting patient-oriented information on hypertension and risk factors, the HHP allows patient to record blood pressure, weight, physical activity and drug consumption..
