Background
Among all health dysfunction patient in Taiwan, 84.4% live with their families. To improve home care quality is now a common goal of our home care team.Methods
We review assessment records from 2015 to 2017. Analyzing all the drug-related problems (DRPs) discovered by pharmacists, then discuss whether the accuracy of medication use can be ..
