Background
Patient’s medication information should be accurately and timely transferred at the point of transfer of care in hospital.Purpose
Establish a care model to improve seamless medication information communicated across our hospital.Methods
We assembled research team of clinical pharmacists, and conducted a literature review about pharmacist.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
