Introduction
Vancomycin requires therapeutic drug monitoring (TDM) to optimise efficacy and minimise nephrotoxicity. This study evaluates the impact of pharmacist intervention on vancomycin use, focusing on pharmacist evaluations, TDM execution, physician acceptance, and intervention types to assess their contribution to therapy.
