BackgroundAtrial fibrillation(AF) is common in the elderly. Oral Anticoagulant (OA) therapy is effective in preventing AF-related embolic events, but the risk of bleeding in patients is increased.
MethodsProspective cohort study with a follow-up of one year was conducted in a medical center in Taiwan. All subjects had two face-to-face pharmacist.. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
