Background
Atrial fibrillation(AF) is common in the elderly. Oral Anticoagulant (OA) therapy is effective in preventing AF-related embolic events, but the risk of bleeding in patients is increased.Methods
Prospective cohort study with a follow-up of one year was conducted in a medical center in Taiwan. All subjects had two face-to-face pharmacist..
