    Back ground: Medication errors are common and often preventable. A study was conducted at a tertiary care hospital for trend analysis of dispensing errors in a hospital pharmacy over past 2 years.


    Objective: objective was to minimize dispensing errors by 25% and enhance patient’s safety through process improvement.
