BackgroundMedicine reconciliation at discharge may reduce errors and readmissions, saving resources and improving quality-of-life.
MethodsThe setting was Karen Grech Rehabilitation Hospital and thirty patients wereinvited to the study. Inclusion criteria were: over 18 years, had a chronic disease and weretaking two or more drugs, were not..
