Medication reconciliation is the process of comparing the patient’s medications before admission to those prescribed during hospitalisation. Accurate and complete medication histories at the time of admission are the critical element of the reconciliation process. Incomplete or inaccurate histories can lead to omissions or inappropriate.. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
