BackgroundA medical center in Taipei faced pharmacist shortage (88 down to 36) since 2017. Excessive workload of Clinical Pharmacists (CP) and overlooked Drug Related Problems (DRPs) indicate a need of a auto-screened toolkit which assist clinical physician providing best patient care. Here we present how CP works with multidisciplinary teams in..
