Background
According to the Annual Report 2017 of Taiwan Patient-safety Reporting System, the percentage of medication errors was found about 35.09% (20,631/58,796) in hospital in this reporting system. In our hospital, the rate of medication error is about 0.0151% in 2017.Methods
