BackgroundUnintentional omissions and delays in medicines dosing are common during hospitalisation, and may occur at any stage of the medication management pathway. Evidence suggests the impact to the patient may vary from insignificant to severe harm. The use of patients’ own medicines (POM’s) has been suggested to improve timeliness of medicines ..
