Objective: To assess the effectiveness of influenza vaccine education and prompting by the Medication Reconciliation (Med Rec) team on increasing vaccination rate at 30 days post hospital discharge.
Method: The med rec team flagged all the admitted patients eligible for vaccination during Dec 2013-Feb 2014 via [quotright]flu bug.. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
