    Background

    Type 2 diabetes patients have a high risk for micro- and macrovascular complications. Hereby, a poor glycemic control leads to progression of disease, hospitalization and reduced life expectancy. In Germany there is lack of evidence regarding a community pharmacist intervention to improve glycemic control of diabetics.
