  • Abstract:

    Background: Medication error may occur in any stage of drug use. Highest rangking occur on prescribing (49%), then administration (26%), dispensing (14%), transcribing (11%). To prevent the occurrence of medication error cooperation, collaboration, and communication between health care professionals through medication reconciliation are necessary
