Background
The Home Medicines Review (HMR) Programme is funded by the Australian government1to enhance patient health and medicines management via accredited pharmacist (AP) and general practitioner collaboration. Little is known about APs’ HMR work processes regarding adverse drug reaction (ADR) reporting.
