BackgroundAn accurate medication history upon hospital admission improves patient health outcomes. When brought into hospital, Patient’s Own Medicines (POMs) provide a source of information for medication history-taking, and have been suggested to increase medication history accuracy.
PurposeThe aim of this study was to evaluate the effect of POMs .. The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
