    Background

    Medication errors have drawn attention from healthcare providers worldwide. Among preventable medication errors, look-alike drugs usually caused unintended pharmacists’ dispensing errors. 

    Purpose

    We presented one serious adverse drug report caused by medication error to remind pharmacist of dispensing look-alike drugs.
