To mark World Heart Day, FIP has launched a new Hypertension Pharmacy Toolkit to support pharmacy teams in the prevention, early detection and management of hypertension.
Affecting one in three adults worldwide, high blood pressure is a major risk factor for cardiovascular disease, which remains the leading cause of death globally. The toolkit equips community pharmacy teams with practical resources to identify at-risk patients, deliver motivational counselling and collaborate with prescribers to improve outcomes. It is built around four key elements: Educational guide on hypertension care; assessment tool for patient counselling and screening; patient leaflet with clear take-home information; and a referral letter to strengthen collaboration with physicians. Additional resources include an infographic and a CPD Bite video demonstrating practical application.
