Introduction: Adverse drug reactions (ADR) are common, causing significant morbidity and mortality. In 2011, there were approximately 247,000 reports of suspected adverse drug reactions reported to the Therapeutics Goods Administration. In 2003, 2-4% of all hospital admissions were medication-related, and up to 75% were potentially preventable,..
