Background: High rates of preventable medication related adverse events have been well documented in primary care systems (PCS) worldwide. Pharmacists are well positioned to prevent many of these adverse events. Comprehensive medication management (CMM) is one pharmacist-led service that has been shown to optimize medication use, but very little..
