Background:
An inadequate discharge planning and ineffective communication in the transition points of patient care contributes to the discontinuation of home care after discharge, and may result in adverse drug events.
Aims:
The relevance and interest in this subject led us to perform this review, in order to analyze and synthesize the available.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
