Persons with Type 2 Diabetes (DM2) have higher rates of cardiovascular disease, are less likely to achieve cardiovascular risk reduction goals and have increased mortality due to cardiovascular disease.
To determine if a pharmacist-led group medical visit program for cardiovascular risk reduction in DM2 added to usual care, could The access to the whole abstract and the presentation file is available to FIP members and to congress participants of that specific congress.
