As Pharmacists we are intrinsically involved with our patient[simplequote]s anticoagulation - managing patients at anti-coagulant clinics, advising clinicians on choice of agent, on prophylaxis versus treatment, and of course always monitoring - PT, APTT, INR, platelets, etc. But to require anticoagulation in the first instance a patient must have..
