The identification of drug therapy problems is one of the main clinical pharmacists' role and is the first step in the process of pharmaceutical care.
The study was performed in the GP office in Split, Croatia where pharmacist worked in collaboration with GP. Study participants were patients with chronic diseases who required drug..
