BackgroundThe UK National Patient Safety Agency (NPSA) stated that 27 deaths and 21,383 incidents were reported concerning omitted doses within secondary care. Omitted medicines can lead to deterioration in health, increased inpatient stays, fewer beds consequently costing thousands.
