Background
Pharmacist case management services to care homes in Northern Ireland (NI) can improve patient outcomes.Methods
Medical and drug history data were retrospectively collected for a sample (n=100) of care home patients from across NI non-electively admitted to acute care. Data was reviewed by case management pharmacists and a decision..
