BackgroundsSodium warfarin (WAR) is a potent anti-coagulant drug with narrow therapeutic index. Time to time dose adjustment is highly important as per the patient therapeutic response. However, precise dose adjustment is not possible with existed WAR solid dosage forms (fixed dose tablets).
