Any primary data collection begins with obtaining necessary and relevant information with a tool; otherwise known as survey instrument or questionnaire. Surveys can be useful in a variety of pharmacy related settings where opinions, attitudes, perceptions, beliefs of human subjects are needed. Results from surveys can be applied in predicting..
