Medicines reconciliation: an essential pharmacist-led service to ensuring patient safety

As the second installment of the two-part series on pharmacists’ role in ensuring patient safety, this webinar will explore medicines reconciliation as an essential service in ensuring patient safety. As transitions of care bear an increased risk of medication discrepancies, which lead to adverse effects, pharmacist-led medicines reconciliation has been demonstrated to effectively resolve such discrepancies and contribute to avoiding patient harm. Pharmacy experts and FIP team members from around the world will be presenting data supporting this service in addition to the FIP toolkit on medicines reconciliation launched this February.
Learning objectives:
• Define medicines reconciliation and review data supporting the clinical and economic benefits associated with the service
• Explore examples of medicines reconciliation practices from different regions
• Become familiar with the FIP medicines reconciliation toolkit to support the advocacy and implementation of the service
