Aims:
To change pharmacy service model and turn passive service into active service, the pharmacists of inpatient pharmacy come into clinic and provide better pharmacy service to medical staff and patients.
Methods:
1.One pharmacist is responsible for four clinical departments. They go into clinical departments and communicate with doctors,.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
