<b>Background Information: </b>To describe the differences in the medication list when a patient ≥ 75 years with Apodos (a specific multi-dose dispensing system) is transferred from the hospital to primary care in the spring of 2009.
<b>Setting:</b> Ystad Hospital, ward 4 and 7.
<b>Methods:</b> The first 25 patients ≥75 years with Apodos .. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
