• Abstract:

  Background

  Therapeutic drug monitoring (TDM) service is one of the pharmacist clinical practices. With TDM, we realize patient's drug compliance and identify drug interactions. There is a TDM service guideline for inpatient, but outpatient is absence in our hospital.
