  Medication errors is a worldwide problem. Many countries have reporting systems, mainly on a small scale, comprising only one hospital or a limited part of a country. Being a monopoly, Apoteket AB (the national corporation of Swedish pharmacies) presents a unique possibility by being able to obtain reports on medication errors from every pharmacy..
