  Background

  There are currently more than 20,000 elderly, high-risk individuals living in rural Eastern Washington who are estimated to have uncontrolled chronic conditions leading to poor population health and increased healthcare expenditure.  Our aim is to develop a sustainable model for pharmacists to provide chronic disease-state management..
