Background
Medication incidents associated with patient harm can either result in sub-optimal disease management or expose patients to unnecessary life-threatening situations.Methods
We extracted 971 medication incidents associated with patient harm from a national incident reporting database in Canada from 2009 to 2017 and performed a qualitative..
