Pharmacist anticoagulation consultation services (ACS) and international normalized ratio (INR) monitoring were implemented in order to reduce drug adverse reactions and improve warfarin therapeutic efficacy.
A retrospective analysis of INR levels between patients taking warfarin who had received ACS and patients who did not was performed. Included
