In the palliative and terminal stages of life, a patient and his informal carers (i.e. family) are faced with complex care problems. In order to effectively tackle these problems, it is important that primary care providers work together as a team. The role of the community pharmacist is essential in this team, because The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
