Introduction
Pharmacists in Germany are required to report adverse drug events including (potential) medication errors (ME), to the Drug Commission of German Pharmacists (AMK). ME are among the most common treatment errors and corresponding AMK reporting forms are available as an online form or PDF files. However, ME reporting rates to the AMK.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
