Introduction
Adverse drug reactions (ADRs) and medication errors are among the leading causes of hospital admissions, particularly affecting vulnerable populations such as the elderly. In the UK, ADR-related hospitalizations cost approximately £770 million annually, while in the US, over 1.5 million hospital admissions per year are linked to..
