Introduction-
Hospital discharge prescriptions are associated with increased risk of medication-related harm to patients. 43% of patients experience unintentional medication errors at discharge and most are put at risk for moderate harm as a result.
Community pharmacists provide patient care by assessing accuracy of discharge prescriptions;.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
