  Background

  A proverb says, “To err is human.” For example, sometimes, the doctor will write an inappropriate prescription owing to the negligence on patient’s liver and kidney function. Therefore, pharmacists will look for any potential medication problems, which can reduce medical negligence and ensure patients’ medication safety.
