The spontaneous individual case safety report remains the golden standard among pharmacovigilance (PV) methods. The success of this method in improving patient safety is yet to be seen in PV systems around the world. At present, significant problems remain with local PV systems. These include fear of potential punitive action, poor PV culture,..
