    Background:

    Adequate patient counseling on their medications use will help to prevent medications errors and achieve patient safety.


    Aims:

    To assess pharmacist-patient counseling in relation to the specified standard guidelines and determine barriers to effective patient counseling.


    Setting:

    Four Government tertiary health facilities in Nigeria.
