The home-dwelling elderly are more at risk for drug related problems because of treatment with polypharmacy, certain drugs can be inappropriate for elderly people and cause adverse reactions such as impaired cognitive function, falls, and gastrointestinal bleeding. They have a higher risk for adverse drug effect or rehospitalization.
