This project is focused on developing the use of concordance in consultations with elderly polypharmacy patients. The aim is to improve the selected polypharmacy patients[simplequote] use of medication, and in addition establishes how pharmacists can contribute through medication review.
Methods
The project includes 9 elderly polypharmacy.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
