  Introduction; Adverse drug reactions (ADRs) contribute significantly to patient's morbidity, mortality & increased health system costs. Studies have reported that cardiovascular drugs are one of the most common causes of adverse drug reactions. It is reported that 20% of outpatients receiving cardiovascular drugs develop an adverse reactions.
