Introduction
The Health Hypertension Passport (HHP) focuses on empowerment, and increase in patients health education, motivation and dialogue between care-partners. Beyond transmitting patient-oriented information on hypertension and risk factors, the HHP allows patient to record blood pressure, weight, physical activity and drug consumption.. The access to the whole abstract and if available the presentation file is available to FIP members and to congress participants of that specific congress.
